Every student in Grades 9,10,11 and 12 will be screened for height, weight, BMI (body mass index) and vision per Pennsylvania School Health Law. New students and all 11th graders must have a physical on file which must include a mandated hearing screening. Any student who fails the vision or hearing screening will be referred to their physician with required follow up if the referral forms are not returned. |

Emergency cards are distributed to all students. Please make sure we have any medical updates on your student and current phone numbers for emergency contacts.

Private Physician Physical Forms are distributed in 10th grade. Physicals must be on file in the nurse's office and can be accepted one year prior to the 11th grade.

New students must provide a copy of their immunizations as required by the PA Dept of Health. Out of state students must provide a physical exam, also.

Water bottles are not permitted at CASHS unless you have a note from your physician for a medical need...please bring any notes to the nurses.

Physical Education is required for graduation. Student Physical Activity Status forms are available in the nurse's office for specific medical concerns and limitations. We can also fax the form to your doctor.

Passes are required (signed by the classroom teacher) to visit the nurses.

Students needing to use the elevator or have extra time between classes for a medical need should have a note from their physician to be given to the nurses who will give you a special elevator/late pass.

Health care insurance in Pennsylvania is available through the CHIPS program. Information and applications for CHIPS are available at the CASHS nurse's office or the PA Dept of Health (see CHIPS website on "useful links").
